This is a friendly reminder that GNTA offers Prescription Co-Payment Reimbursement and the due date for claims is March 31, 2017.
Through the GNTA Benefit Trust Fund members are entitled to reimbursement for co-pays incurred during the 2016 calendar year for prescription drugs up to a maximum of $250, plus $1 for each additional prescription over the $250 amount. This benefit is limited to one reimbursement per member, including dependents (not one per family member). To access the benefit, go on the GNPS website to Forms on the sidebar. Click GNTA, then go to Active Prescription Claim Form. The directions are on the form and are easy to follow; and the form must be submitted no later than March 31, 2017.
